In a small number of patients, capsular phimosis can happen after cataract surgery. This can lead to severe distortion of the IOL optic and this results in poor vision for the patient. Some of the underlying conditions which make phimosis more likely are pseudo-exfoliation and retinitis pigmentosa.
The small circular opening seen in the photo is the capsulorhexis which has shrunk from 5 mm down to about 2 mm in diameter. The radial lines are the areas of stretched zonular support and the IOL has also decentered.
