Dr. Sandra Lee, M.D., better known as Dr. Pimple Popper, has covered a lot of territory in her popping videos. She’s conquered bowling-ball sized cysts from a man’s neck, removed a “cheesy” cyst, and gotten rid of more “bacne” that one could imagine. But now she’s moving into the super-sensitive world of eyelids, and naturally, filming it all for her loyal fans.

In her latest video a woman, who has tons of whitehead-like bumps all across her thin-skinned lids and beneath her eye, visits the internet-famous dermatologist. The bumps are actually milia and the patient says she got them after “the worst allergy attack” she’d ever had, a few years prior. You can watch the full video here:

“[Milia] bumps are hard, white cysts under the skin,” board-certified dermatologist Joel Schlessinger previously explained to Women’s Health. “Picking at them often has little to no effect and attempting to pop them will likely leave your skin red, irritated, and inflamed, with the milia still intact.” He explained to get them extracted, one would have to see a dermatologist to have them removed. (Dealing with run-of-the-mill acne? In this video, the patient receives a local anesthesia to the area before Dr. Pimple Popper goes in with a super-sharp pointed blade (the blade, so close to the eye, will no doubt make you nervous). She starts working on the milia one by one, which often results in blood. In some cases, the blade alone is not successful, and the doctor has to use tiny scissors to remove them, again, incredibly close to the eyeball. Since each bump needs to be removed individually, and they can be finicky, the procedure is lengthy.

While the video is over 20 minutes long, at the end Lee says the procedure actually took a whole hour! And she hopes it’ll be continued, as there is still more work to be done, since this woman’s milia is very extensive.
